Ich bin damit einverstanden, dass das Hotel zum Schutz der öffentlichen Gesundheit aktualisierte Unterlagen aller Gäste, die sich im Hotel aufhalten, bewahren muss - Name, Nationalität, An- und Abreisedatum, Kontaktdaten (Telefon, E-Mail), damit einen Kontakt möglich ist, falls zu einem späteren Zeitpunkt ein Coronavirus-Fall festgestellt wird. Die Datenschutz-Grundverordnung wird von allen Mitarbeitern eingehalten und die Angaben der Gäste werden aus Gründen des Gesundheitsschutzes in Dateien gespeichert.

Zgadzam się, że w celu ochrony zdrowia publicznego hotel musi na prowadzić aktualny zapis wszystkich gości przebywających w hotelu – imię i nazwisko, narodowość, datę przyjazdu i wyjazdu, dane kontaktowe (telefon, adres e-mail), tak aby komunikacja była możliwa w przypadku koronawirusa w późniejszym terminie. RODO jest przestrzegane przez wszystkich pracowników, a informacje o gościach będą przechowywane w aktach ze względu na ochronę zdrowia publicznego.

Ich erkläre und bestätige, dass ich ordnungsgemäß darüber informiert wurde, dass das KOALA HOTEL die gesetzlichen Bestimmungen bezüglich der speziellen Gesundheitsprotokolle erfüllt (gemäß dem Gemeinsamen Ministerialbeschluss 1881/29.5.2020 bezüglich der speziellen Gesundheitsprotokolle, die für die Tourismusunternehmen im Hinblick auf die Maßnahmen gegen Covid-19 gelten werden) und erkenne an, dass das Hotel nicht haftbar gemacht werden kann (für die Wiederherstellung von Schäden oder anderweitig), solange die in den besagten Gesundheitsprotokollen vorgesehenen Maßnahmen eingehalten werden.

Dichiaro e confermo di essere stato debitamente informato che KOALA HOTEL soddisfa le disposizioni di legge in materia di protocolli sanitari speciali (come previsto dalla Decisione Ministeriale Congiunta 1881 / 29.5.2020 relativa ai protocolli sanitari speciali che si applicheranno alle imprese turistiche alla luce delle disposizioni contro Covid-19) e riconosco che l'Hotel non potrà essere ritenuto responsabile (per il ripristino di danni o altro), purché siano rispettate le misure previste da detti protocolli sanitari.

Oświadczam i potwierdzam, że zostałem należycie poinformowany, że KOALA HOTEL spełnia przepisy ustawy dotyczące specjalnych protokołów opieki zdrowotnej (zgodnie ze wspólną decyzją ministerialną 1881/29.5.2020 w sprawie specjalnego protokołu opieki zdrowotnej które będą miały zastosowanie do przedsiębiorstw turystycznych w świetle środków przeciwko Covid-19) i uznają, że Hotel nie ponosi odpowiedzialności (za przywrócenie szkód lub w inny sposób), o ile środki przewidziane w tych protokołach opieki zdrowotnej są przestrzegane.

Ich bestätige, dass ich und der/die Mitbewohner meines Zimmers keine Symptome der Covid-19-Krankheit haben, dass wir nicht in Quarantäne sind, dass wir alle notwendigen Vorsichtsmaßnahmen gegen die Epidemie ergreifen und dass wir auf eigenes Risiko reisen.

Confermo che io e chi condivide la mia stanza non abbiamo sintomi della malattia Covid-19, non siamo in quarantena, prenderemo tutte le precauzioni necessarie contro l'epidemia e che viaggiamo a nostro rischio.

Potwierdzam, że ja i współdzielący mojego pokoju nie mają objawów choroby Covid-19, nie jesteśmy w kwarantannie, podejmiemy wszelkie niezbędne środki ostrożności przed epidemią i podróżujemy na własne ryzyko.

I agree that for public health protection, the hotel must keep an updated record of all guests staying at the hotel – name, nationality, date of arrival and departure, contact details (telephone, e-mail), so that communication is possible if a coronavirus case is identified at a later time. The GDPR is complied by all staff members and guests information will be kept on file for reasons of public health protection.

In Bezug auf die Voraussetzungen der EU-Verordnung 2016/679 (Datenschutz-Grundverordnung) stimme ich der Verarbeitung meiner personenbezogenen Daten gemäß der Datenschutzerklärung zu.

Συμφωνώ Για την προστασία της δημόσιας υγείας, το ξενοδοχείο πρέπει να τηρεί ενημερωμένο αρχείο όλων των επισκεπτών που διαμένουν στο ξενοδοχείο - όνομα, εθνικότητα, ημερομηνία άφιξης και αναχώρησης, στοιχεία επικοινωνίας (τηλέφωνο, e-mail), έτσι ώστε η επικοινωνία να είναι δυνατή σε περίπτωση ιχνηλάτησης κρούσματος covid-19 και μετά την αναχώρηση του επισκέπτη. Όλο το προσωπικό συμμορφώνεται πλήρως με τις Αρχές Προστασίας Προσωπικών Δεδομένων και οι πληροφορίες των επισκεπτών θα διατηρούνται σε αρχείο για λόγους προστασίας της δημόσιας υγείας.

Accetto che per la protezione della salute pubblica, l'hotel debba tenere un registro aggiornato di tutti gli ospiti che soggiornano presso l'hotel - nome, nazionalità, data di arrivo e partenza, dettagli di contatto (telefono, e-mail), in modo che sia possibile la comunicazione se un caso di coronavirus viene identificato in un secondo momento. Il GDPR è rispettato da tutti i membri del personale e le informazioni sugli ospiti saranno conservate per motivi di protezione della salute pubblica.
